Athletic Clearance
Every student wishing to physically participate in any Oakmont Athletics activities, tryouts, practices, or contests must be cleared by OHS Athletics. As a part of the clearance process, every athlete is required to have a yearly physical from a qualified physician. The physical must be completed ON OR AFTER MAY 15, 2024. Any physicals dated before then are no longer eligible for the 24-25 seasons. Please follow the directions below to complete the online clearance process.
We do not accept paper copies of forms for the clearance process. All Physical and Health History forms must be uploaded to the clearance site. Oakmont Main Office has computers and can support families with this online process.
Online Athletics Clearance Process
- Print and bring the Health History and Physical Exam Form, linked in the button above, to your doctor to complete. Make sure to bring the Health History and Physical Exam Form with you to your appointment.
- Get your athlete's STUDENT ID # from them or your Homelink account. If you are an incoming 9th grader and do not have access to your Homelink account, please enter "000000".
- Start the online process at Athletic Clearance as soon as possible by clicking on the link or the button labeled "Athletics Clearance Site". http://www.athleticclearance.com
- Register for an account – using the parent’s email address
- Once registered, find the Start Clearance Here tab – if you have more than one student-athlete attending OHS, you will need to add clearance for each. If your student athlete plans on playing more than one sport, you will click on all sports your student athlete plans on playing. Fill in all the information on the student information page.
- ImPact Concussion Baseline Testing - If you have not taken your Impact Baseline at all, or it has been 2 school years since your last baseline test, you must follow the directions and complete the ImPact Baseline test on your computer at home, then upload a picture or screenshot of your completion page. If you completed the baseline test last school year (23-24), you can upload a picture or screenshot of your previous completion page. If you are unsure or cannot access your unexpired completion page, please take it again!
- UPLOAD ALL 4 PAGES of the completed Health History forms and Physical Exam Forms to the site toward the bottom of the student information page. Please make sure not to load it just into the library, but into the spot for the physical itself.
- If you have a physical that has not reached its expiration, you can upload it for summer or off-season purposes. All physicals must be dated on or after May 15, 2024, before the official sports season begins, no matter if they are expired or not.
- Complete the Parent/Guardian Information page
- The athlete AND the parent will need to electronically “sign” the required sections as prompted.
- Submit the clearance.
- You will receive a confirmation email when everything has been submitted and one when your student-athlete has been CLEARED by the school to participate that explains your final steps to get cleared.
Note
- You must have insurance or purchase school insurance for your student-athlete to be cleared.
- If you are entering the 10th, 11th or 12th grade, and you attended a different high school other than Oakmont last year, you will need to complete the questions for being a transfer. Email Athletic Director, Tim Moore, immediately so we can begin the CIF/SJS mandated transfer process. This will need to be completed before you can compete against another school for Oakmont.
- PLEASE NOTE THAT ONLY THE PROVIDED RJUHSD FORMS WILL BE ACCEPTED FOR CLEARANCE PER RJUHSD POLICIES.
- FORMS SHOULD BE TAKEN WITH YOU TO YOUR PHYSICAL APPOINTMENT.
Oakmont Athletics is going paperless
All our coaches now have the HOME CAMPUS app, so all athletic clearance's and emergency information is on their cell phone. Once you have received your confirmation email saying that you have completed all the steps for your athletic clearance, the coach will be notified on their app that you are athletically cleared and ready to begin full participation in your sport. Athletes do not need to provide their coach with their medical emergency form, formerly known as the "Ticket to Tryout".
Physicals
Physicals must be dated on or after May 15, 2024. Please upload the completed form to the Athletic Clearance as part of the athletic clearance process.

Academic:
2.0 GPA and pass 3 classes during the last grading period
Citizenship:
Clear all fines and detentions; meet training rules; abide by handbook guidelines
Residential:
Our transfer students and their families must complete additional forms and meet with the Athletic Director, Tim Moore - tmoore@rjuhsd.us, to get cleared by both OHS Athletics and the CIF/SJS before they will be allowed to compete against another school.
